Learning Objectives:


This half-day training will provide an understanding of a contemporary model of Nephrology Social Work practice that emphasizes the strategic and focused delivery of social work services to target specific psychosocial variables that influence patient survival and quality of life
. 

 

Learning competencies:

 

1.       Participants will be able to identify the value of delivering individual and group social work interventions that objectively measure improved outcomes in areas such as adherence, rehabilitation, reduced depression and patient satisfaction with care.

 

2.       Participants will be able to identify barriers to reorganizing their daily NSW practice into an outcomes-driven model.

 

3.       Participants will be able to design and propose brief, strategic social work interventions to improve patient treatment outcomes and monitor the effectiveness of those interventions.

 

Presenter:

Stephanie Johnstone, LCSW

Ms. Johnstone is a practicing nephrology social worker.  She has written and spoken extensively on the topic of improving CKD patient outcomes through the use of an out-comes driven model of social work practice.
